The Problem
Staff responsible for compliance documentation were struggling to complete records on time. This left leadership uncertain whether essential actions had been taken and without clear records, there was no way to demonstrate actions were taken.

The Risk
Incomplete documentation created organizational risk by undermining both compliance and safety. Without reliable records, the organization could not confirm whether compliance issues were addressed or if actions were taken.

The Approach
To uncover root causes and opportunities for improvement, I guided the organization through a multi-step process:

1. Staff Survey – Designed and launched a confidential survey to assess staff process experience, motivations, and barriers. Responses were grouped into seven categories:

2. Root Cause Analysis – Created a fishbone diagram to categorize issues and identify quick wins. Quick wins included removing outdated or repetitive form questions, simplifying wording, and ensuring workflow notifications worked as intended.

3. Process Mapping – Built a swimlane map to connect each step with its purpose, creating clarity and sparking discussion on where changes would add the most value.

4. Collaborative Solution Sessions – Facilitated meetings with staff to co-design improvements, ensuring buy-in and gathering practical solutions from the people closest to the work.

5. Updated Procedures – Documented and gained approval for the new process, ensuring clarity and consistency.

6. Follow-Up & Accountability – Incorporated monitoring and feedback loops to reinforce the new process and correct early missteps.

The Results
Within three months, incomplete documentation dropped from an average of 185 per day to just 40, a 78% reduction.
